INDUCTION CEREMONY CONSENT FORM

This consent form applies to both adults and children who are participating in our Induction Ceremony. We request permission to use your name, comments and photo to be published on the Yellow Rose-Strong Oak Society’s website and other social media associated with the Yellow, Blue Diamond Society and Half Century Club. If you have more than one child participating, we need a consent form for each of them. If you wish to rescind this agreement, you may do so at any time in writing, prior to the event, by sending a letter to the facilitator of this event. The law requires that we receive your permission to use your image or name before doing so. The Yellow Rose-Strong Oak Society, Blue Diamond Society or Half Century Club wants to celebrate the participants of both children and adults by placing them on our website.

Please Print your full name to grant permission for my child/ myself to be photographed and have their name, comments and image published on the Yellow Rose-Strong Oak Society public as well as other internet sites associated with the Yellow Rose-Strong Oak Society.

INDUCTION CEREMONY APPLICATION

What we will need two months before the birthday of your recipient:

  • A current photo of the inductee in the form of a .jpg file sent to info@yrsos.org.

  • Brief biography of the potential inductee sent to info@yrsos.org.

  • Payment of $70 to the YRSOS PayPal account (Click button below to complete your payment)



YRSOS Calendar Checklist

The following checklist is designed with the (ideal) timetable of twelve months. If you’re planning your Yellow Rose Strong-Oak Society Induction Ceremony on a shorter schedule, just start at the beginning of the list and try to catch up as quickly as possible. Use the boxes to the left of the items to check off tasks as you complete them.  The ceremony is in addition to the celebration planned by the family. Both events can be as grand or simple as the family would like. This is a guide for both.

9 to 12 months prior to Birthday:

  • Arrange a gathering for you and the family of the inductee

  • Determine the budget and how expenses will be shared.

  • Discuss the size, style, location, and scope of the Yellow Rose Strong-Oak Society Induction Ceremony you want.

  • Choose a target Yellow Rose Strong-Oak Society Induction Ceremony date and time. (The actual date will depend on venue availability.)

  • Create a binder to store and organize ideas, worksheets, receipts, brochures, etc.

  • Visit and reserve Yellow Rose Strong-Oak Society Induction Ceremony.

  • Start compiling your guest list to estimate head count. Consider budget when thinking about “must-invites” versus “nice-to-invites.”

  • Hire a photographer and a videographer

  • Hire a caterer.

4 to 6 months prior:

  • Finalize the guest list.
