INDUCTION CEREMONY CONSENT FORM

This consent form applies to all who are participating in our Induction Ceremony. We request permission to use your name, comments, and likeness to be published on the Yellow Rose-Strong Oak Society’s website and other social media associated with the Yellow, Blue Diamond Society, and Half Century Club. We need a consent form for each individual. The Yellow Rose-Strong Oak Society, Blue Diamond Society, or Half Century Club wants to highlight everyone who made this celebration happen.

A current photo and a brief biography of the inductee sent to info@yrsos.org.
Payment of $70 to YRSOS via Zelle or PayPal account.
